This field-based positionoffers clinical autonomy while working alongside an interdisciplinary care teamfocused on improving patient outcomes, reducing hospital readmissions, andsupporting patients as they recover safely in the comfort of their homes.

Compensation & Benefits:
- Base salary $119,313 -$178,968 annual

Qualifications:
- Master of Science in Nursing
- Adult/Geriatric NursePractitioner certification
- Active, unrestrictedCalifornia RN license
- Active, unrestrictedCalifornia NP license
- Minimum of 1 year of NursePractitioner clinical experience
- Strong assessment,diagnostic, and care management skills
- Valid driver's license andreliable transportation
- Ability to workindependently while collaborating with a multidisciplinary team
- Current TB test orwillingness to obtain prior to employment
