Provide comprehensive primary care to adult patients in a general family practice, with an emphasis on prevention, chronic disease management, acute same-day visits, and women’s health. Collaborate with assigned physicians and a multidisciplinary care team under a collaborative practice agreement, consulting physicians on complex cases.

Responsibilities

• Assess adult patients and develop differential diagnoses.
• Diagnose conditions and create individualized treatment plans.
• Manage prescriptions in accordance with the collaborative practice agreement.
• Deliver preventive care and conduct health screenings.
• Manage chronic diseases and provide acute, same-day care.
• Provide women’s health services.
• Educate patients on conditions, treatments, and preventive health.
• Coordinate care with assigned physicians and the multidisciplinary care team.
• Maintain accurate clinical documentation in electronic health records.
• Consult physicians regarding complex cases.

Required Qualifications

• Active Nurse Practitioner license.
• At least 2 years of experience in family practice nursing.
• Knowledge of family medicine, differential diagnosis, pharmacology, chronic disease management, and preventive care.
• Proficiency with electronic health records.
• Strong clinical judgment and patient communication skills.
