Responsibilities:
Provide direct patient care, including assisting with ADLs, vital signs, and other basic care needs
Develop and implement personalized care plans in collaboration with the interdisciplinary team
Maintain accurate and detailed records of patient care and treatment
Participate in medication management, including administration and monitoring of medications
Foster a positive and supportive environment for patients, families, and staff
Collaborate with other healthcare professionals to ensure seamless care delivery
