Job Title: LPN / LVN – LTC
Location: Utica, NY
Job Type: Contract
Duration: 13 Weeks
Shift: Evening | 2:00 PM–10:00 PM
Schedule: 5 days/week | 8 hours/shift | 37.5 guaranteed hours/week

Local: $33.00 - $36.00/hour

We are seeking an experienced LPN/LVN to provide nursing care to residents in a Long-Term Care setting.
Required QualificationsActive New York State LPN License
Current BLS Certification
Minimum 1 year of LTC experience
Recent Long-Term Care facility experience
Provide direct nursing care to LTC residents.
Assess residents and monitor changes in condition.
Administer medications and treatments according to orders and facility protocols.
Assist with individualized care plans.
Document patient care, assessments, medications, and interventions in PointClickCare EMR.
Care for residents with MS, orthopedic conditions, dementia, and memory-care needs.
Communicate changes in resident condition to the healthcare team.
Collaborate with CNAs, RNs, physicians, and other healthcare professionals.
Follow infection-control, safety, and facility policies.
Maintain accurate clinical documentation and regulatory compliance.
Float within the same building as needed.
Work every other weekend.
Perform other LPN/LVN duties as assigned.
