We are hiring a Full-Time Family Nurse Practitioner (FNP) for a primary care clinic in San Jose, CA. Required Qualifications
- Master's or Doctorate from an accredited Nurse Practitioner program
- Active California Nurse Practitioner License
- Board Certified by the California Board of Registered Nursing
- Active, unrestricted DEA License
- Current BLS Certification
- Minimum 2 years of Primary Care clinical experience
- Community Health Center or FQHC experience preferred
Responsibilities
- Provide comprehensive primary care, including assessments, diagnosis, and treatment
- Perform physical exams and obtain medical histories
- Prescribe and manage medications
- Develop individualized treatment plans
- Educate patients on disease prevention and wellness
- Coordinate referrals to specialists
- Collaborate with physicians and interdisciplinary teams
- Maintain accurate EHR documentation
- Participate in team meetings and after-hours on-call rotation
