Family Medicine Nurse Practitioner or Physician Assistant (Outpatient)
Ready to deliver full-scope family medicine to patients of all ages in a mission-driven setting? An established outpatient practice in the New York, NY area is hiring a Nurse Practitioner or Physician Assistant to provide primary care across multiple community sites!

Schedule
- Full-time, Monday-Friday, 9:00am-5:00pm
- Floats between multiple sites (Bronx and Brooklyn)
- Onsite (this is not a remote or hybrid role)
Compensation
- $150,000 base salary, plus a per-patient incentive above a daily threshold
What You’ll Do
Provide comprehensive primary care to pediatric, adolescent, and adult patients:
- Take patient histories, identify needs, and make diagnoses
- Develop and implement treatment plans using diagnostic data
- Perform well-care and physical examinations, including gynecological exams as indicated
- Write prescriptions and counsel patients on medications
- Make and track referrals with follow-up care
- Assist with immunizations and phlebotomy as needed to support patient flow
- Provide health education on preventive care, chronic disease, prenatal, nutrition, and family planning
- Collaborate with physicians and specialists; supervise medical assistants on-site
What You Bring
- PA or NP with an unrestricted New York State license (NP must be FNP to cover the full age range)
- Family medicine experience (infants to adults)
- Ability to float between all sites
- Spanish or other languages a plus
