Full-Time & PRN Physical Therapist (PT)

PRN Opportunities $60/hr.

Evaluate residents and develop individualized physical therapy plans of care
Provide skilled PT services focused on mobility, strength, balance, endurance, safety, and independence
Establish goals and monitor resident progress
Help residents achieve their highest possible level of function
Complete accurate and timely documentation using Net Health
Utilize PointClickCare (PCC) as appropriate
Collaborate with nursing, therapy, physicians, families, and the interdisciplinary team
Provide supervision and clinical guidance to PTAs as appropriate
Participate in discharge planning and promote safe transitions
Maintain compliance with applicable state, federal, facility, and professional standards
Graduate of an accredited Physical Therapy program
Current New York Physical Therapist license or eligibility for NY licensure
SNF/LTC experience preferred, but we welcome qualified candidates from a variety of backgrounds
Strong communication and organizational skills
Positive, team-oriented approach
