We are looking for a passionate and dedicated healthcare professional to provide exceptional patient care in a supportive and collaborative environment.Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans.
- Perform diagnostic tests and interpret results to guide treatment decisions.
- Administer medications and provide patient education on health management.
- Collaborate with multidisciplinary teams to ensure holistic patient care.
- Maintain accurate and timely documentation of patient interactions.
- Stay current with best practices and contribute to quality improvement initiatives.
- Participate in community outreach programs to promote health awareness.
- Provide compassionate support and guidance to patients and their families.
- Current FNP-C certification and a valid Colorado nursing license.
- Minimum of 3 years of clinical experience in primary care family practice or a related field.
- Strong communication skills and a patient-centered approach.
- Ability to work effectively in a fast-paced environment.
- Commitment to continuous learning and professional development.
- Experience with electronic health records (EHR) systems. EPIC experience helpful.
- Demonstrated ability to build rapport with diverse patient populations.
- Willingness to participate in team-based care and community initiatives.
