Clinical Team Coordinator

Emmanuel Hospice is seeking an experienced and compassionate Clinical Team Coordinator to provide leadership and operational oversight for interdisciplinary hospice care teams. This role is responsible for ensuring exceptional patient care, efficient clinical operations, regulatory compliance, staff development, and collaboration across disciplines. The Clinical Team Coordinator plays a critical role in advancing our mission of delivering compassionate, patient-centered end-of-life care

Qualifications:

- Valid Michigan licensure as a Registered Nurse (RN),BSNpreferred
- Minimum of 5 years of healthcare experiencewith hospice experience preferred
- Previousleadership,managementor supervisory experiencepreferred
- Proficiencywith Electronic Medical Records (EMR) systems and Microsoft Office applications.

Responsibilities include:

- Oversee daily clinical services to ensuretimelypatient care andappropriate utilizationof clinical resources.
- Monitor staffing coverage, visit completion, and workflow challenges, addressing operational issues promptly and effectively.
- Partner with Scheduling tomaintainadequate staffing andoptimizeclinical productivity.
- Lead daily team prioritizationand staff performance efforts to support quality care, service excellence, and productivity goals.
- Ensure consistent implementation of individualized care plans and adherence to hospice standards across interdisciplinary teams.
- Facilitate and lead Interdisciplinary Team (IDT) meetings.
- Promoteaccountability,timelycare and communication,high-qualitydocumentation,and professionalpractice.
- Conduct ride-alongs, shadowing, coaching sessions, and chart reviews to support professional growth and clinical excellence.
- Supportstaffthroughcoaching,feedback,andongoingperformance improvementinitiatives.
- Ensure clinical services are available 24 hours per day, 7 days per week, including participation in a manager on-call rotation and providing clinical support as licensure permits.
- Collaborate with physicians, the Medical Director,Director ofPatientCare,andinterdisciplinary team members tocoordinatepatientcareandsupportpatientandfamilygoals.
- Build andmaintainstrong relationships with healthcare facilities, referral partners, and community stakeholders. xxzpjkt
