Nurse Practitioner ~ Full Time

Join Riverside Home Health & Hospice and make a difference in the lives of patients throughout our community. We are seeking a compassionate and motivated Nurse Practitioner to provide high-quality mobile and community-based healthcare services.
What You'll Do✅ Diagnose and treat acute and chronic conditions
✅ Provide care in homes, Assisted Living Facilities (ALF), and Skilled Nursing Facilities (SNF)
✅ Conduct patient assessments, develop treatment plans, and prescribe medications
✅ Utilize telehealth and in-person visits
✅ Collaborate with patients, families, and healthcare partners
✅ Maintain accurate and timely clinical documentation
✔ Master's Degree from an accredited NP program
✔ Current state licensure and prescribing authority
✔ Valid driver's license and reliable transportation
✔ Strong communication and clinical decision-making skills
✔ Knowledge of HIPAA and healthcare regulations
