Key Responsibilities

  • Conduct detailed assessments of patients to identify emotional and behavioral issues, establishing rapport and trust in a remote setting.
  • Collaborate with a multidisciplinary team of healthcare providers by communicating patient progress, treatment needs, and any concerns to ensure comprehensive care.
  • Maintain thorough and accurate clinical records, including treatment plans and progress notes, adhering to established protocols and department policies.
  • Develop and implement individualized treatment plans based on patients' psychosocial assessments and evidence-based practices.
  • Provide crisis intervention and support to patients experiencing acute emotional distress, utilizing de-escalation techniques and therapeutic interventions.
  • Assist patients with discharge planning, ensuring smooth transitions to other levels of care or outpatient services, coordinating with a range of community resources.
  • Engage and consult with family members and support systems regarding client treatment and discharge coordination, fostering a holistic approach to mental health.

Patient Population / Client Focus

Your expertise will focus on a diverse clientele, including individuals struggling with anxiety, depression, trauma, and other mental health disorders. You will engage with patients from various age groups, backgrounds, and socioeconomic statuses, tailoring your therapeutic approach to meet their unique needs.

Qualifications

  • A Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology is required.
  • Current state licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or equivalent is essential.
  • Minimum of three years of experience working in an outpatient mental health setting, demonstrating proficiency in clinical practices.
  • Expertise in delivering evidence-based treatments and mental health services, with a comprehensive understanding of various therapeutic modalities.
  • Strong organizational skills and the ability to work independently in a remote environment, managing time effectively and maintaining high standards of care.
  • Proficient in utilizing technology and telehealth platforms, as well as standard office software (e.g., MS Office).

Preferred Experience

While not mandatory, candidates with experience in integrated care models or those who possess specialized training in cognitive-behavioral therapy (CBT) or dialectical behavior therapy (DBT) will be given preference. Familiarity with electronic health record (EHR) systems and telehealth technologies is also advantageous.
