Job Summary
Carease Health - is a nationwide preventative healthcare company focused on reducing hospitalizations. We are dedicated to improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care. As a Nurse Practitioner (NP) or Physician Assistant (PA), you will play a crucial role in delivering high-quality preventative care to residents in skilled nursing facilities.
About the Role
We are seeking an experienced NP or PA with a background in Geriatrics or Internal Medicine to join our team. As a hybrid role, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player. You will work closely with our clinical directors, regional managers, and supervisors to provide exceptional care and support.
Responsibilities
Provide high-quality preventative care to residents in skilled nursing facilities
Conduct on-site visits and remote work from home, supported by a dedicated scribe team
Build strong relationships with facility staff and maintain a patient-centered approach
Collaborate with our clinical team to ensure seamless care coordination
Participate in quality improvement initiatives to reduce hospitalizations and improve patient outcomes
Maintain accurate and timely documentation of patient care

Requirements
Active NP or PA license (in the state of Pennsylvania)
1+ year of experience in:
Geriatric care
Documentation review
Excellent communication, collaboration, and teamwork skills
