About this position

Opportunity Highlights- Schedule: Monday-Friday (3-4 days per week will also be considered); 8:00am-5:00pm; minimum 2 weeks per month (full-time preferred); start date November 2026, ongoing
- Job Setting: Outpatient clinic; includes in-clinic and community/home visits (assisted living, SNF, or personal residence)
- Types of Cases: Geriatric patient population (adults 55+); PACE program (Program of All-Inclusive Care for the Elderly); interdisciplinary team-based care; 4-6 patients per shift; community visits conducted with a team nurse
- Credentialing: Estimated credentialing timeframe: 0 days; fellowship required; DEA required; active OR license required; no temporary privileges available
- EMR: Epic
- Board Certified/Board Eligible: Board Certified or truly Board Eligible required; geriatric experience strongly preferred
- Certifications: BLS required
- Licensure: Active Oregon license required; IMLC not accepted
